High risk medicine safety information survey

This survey has been developed to assist in evaluating the impact of High Risk Medicines Working Party alerts on patient safety processes. After completing the survey, users may return to the beginning to provide feedback on a different alert.

The information provided to us will be used to refine and improve future safety alerts and bulletins and to tailor them to the needs of health services. All replies will be confidential.
